The Serious Accident Review Team (SART) Member Training Program will prepare the student to perform as a Serious Accident Review Team member tasked with reviewing firefighter injuries and line of duty deaths. The student will gain understanding of investigating accidents in a complex system. This class uses the new understanding of “human error” as the basis for the investigation process. Topics covered will include:

• Laws and Regulations
• Union Issues and Firefighter Bill of Rights
• Just Culture
• Agency Roles, Team Organization
• Initial Scene Processing and Evidence Collection
• Scene Photography and Diagramming
• PPE Inspection and Documentation
• Cognitive Interview Process
• Human Error
• Analyzing Accident Data
• Telling the Story
• Developing Written Reports

 

INSTRUCTORS
Mike Teague, Captain, Sacramento Metro FD, Retired
Captain Teague has over 30 years of experience in fire and emergency medical services. He is a member of the Serious Accident Review Team investigating significant line of duty injuries and deaths, and was the principle author of several of the reports. Capt. Teague provides instruction for Line of Duty Death/Injury and Near Miss Investigation local and nationally. He has spoken at FDIC on the topic and has been published in Fire Engineering magazine.
Shawn Pruchnicki, MS, RPh, ATP, CFII
Shawn is a faculty member at Ohio State University (OSU), where he teaches aviation safety, human factors, accident investigation, and complex aircraft operation for the Department of Aviation. He was a firefighter/paramedic with Jackson Township in Grove City, Ohio. His research focus includes the role that safety culture, human performance, and human error play in understanding accident causation. He is pursuing a PhD in cognitive engineering at OSU.
